| 2. Print and complete the Mail/Fax Order Form. |
| • Include shipping charge based on rates for your desired shipping method as specified on the Shipping Info page. |
| • If your order is to be shipped to an address in the state of New York, include 4.625% sales tax on each item costing less than $110 and 8.625% on each item costing $110.00 or more. |
| 3. Mail or fax the form and payment as specified in #4 above. |
